Conduct initial intakes and assessments with children in grades K-12 and their parent/guardian
Collaborate with other Hazel Mental Health Therapists to ensure children successfully transition into services
May provide ongoing virtual counseling and solution-focused therapeutic interventions for children in grades K-12 as needed
Provide culturally relevant and age-appropriate counseling and treatment
Collaborate with Hazel’s care management team to identify and support transition services
Maintain relationships and engage with school personnel, family members, and stakeholders
Obtain necessary state- or district-required credentialing as requested by Hazel’s credentialing department
Manage own caseload, ensuring timely intakes, assessments, and follow-up sessions
Be responsive to communication from school personnel, families, and care management team and participate in occasional virtual meetings or trainings
Participate in 15 hours of paid training during initial onboarding and ongoing as needed
Time spent on insurance credentialing is reimbursable
Maintain required equipment, internet, and workspace to perform telehealth services securely and without lag
Requirements
Licensed without restrictions as a Licensed Independent Social Worker (LISW), Licensed Marriage and Family Therapist (LMFT), or Licensed Mental Health Counselor (LMHC)
Licensed in LA for a minimum of 3 years
Additional license in AL, CA, CO, FL, GA, HI, MA, MD, MO, NV, TX, VA, or WA; or be eligible and able to begin a state license application process immediately upon signing contract
Willingness/ability to be enrolled by Hazel with insurance companies, including Medicaid and private payers
Possess a working knowledge of treatment strategies and interventions related to the mental health needs of children and their families
Demonstrates an ability to make accurate and timely clinical judgments or recommendations
Works in collaboration with other clinical team members across Hazel Health
Demonstrates experience and education in all areas of clinical practice
Highly proficient in computer and technology skills, including ability to use a laptop, quickly learn Hazel’s clinical platform, navigate Google and Slack, learn new software, and complete digital paperwork
Maintain a HIPAA-compliant private, unshared, and secure workspace
Acquire malpractice insurance providing a minimum of $1 million per claim and $3 million in aggregate coverage
Computer with webcam, microphone, speakers/headset
Stable home internet connection with at least 20 mbps
Up-to-date operating system (e.g., supported Windows or macOS) and Google Chrome installed
Availability Monday–Friday after 4pm CST (current need)
Minimum weekly time commitment based on caseload requirements (to be discussed during onboarding)
