About the role
Key responsibilities & impact- Provide in-home primary care to high-acuity patients
- See approximately 6–9 patients per day with 30–45 minute visits
- Manage a patient panel across Denton, Aubrey, Lewisville, and Carrollton areas
- Focus on patient care rather than visit volume
- Take the time needed to address patients’ needs
- Practice within an integrated, team-based, physician-driven value-based care model
- Work Monday–Friday with no weekends or evenings
- Perform home-based primary care with minimal call responsibilities
Requirements
What you’ll need- Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located
- Federal DEA license or eligibility
- Active CPR/BLS certification
- Board certification
- Valid driver’s license and good driving record
- Directly related experience in home health, geriatrics, hospice, or primary care
- Geriatric training/experience preferred
