About the role

Key responsibilities & impact
  • Provide in-home primary care to high-acuity patients
  • See approximately 6–9 patients per day with 30–45 minute visits
  • Manage a patient panel across Denton, Aubrey, Lewisville, and Carrollton areas
  • Focus on patient care rather than visit volume
  • Take the time needed to address patients’ needs
  • Practice within an integrated, team-based, physician-driven value-based care model
  • Work Monday–Friday with no weekends or evenings
  • Perform home-based primary care with minimal call responsibilities

Requirements

What you’ll need
  • Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located
  • Federal DEA license or eligibility
  • Active CPR/BLS certification
  • Board certification
  • Valid driver’s license and good driving record
  • Directly related experience in home health, geriatrics, hospice, or primary care
  • Geriatric training/experience preferred
