About the role
Key responsibilities & impact- Provide in-home primary care to high-acuity patients
- Practice medicine with a focus on patient care rather than volume
- Take the time needed to address patients’ needs
- Work Monday through Friday
- Practice within an integrated, team-based, physician-driven model
- Conduct appointments with assistant support in select markets
Requirements
What you’ll need- Licensed as a NP in the state where the HarmonyCares Medical Group office is located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Valid driver’s license and good driving record
- Directly related experience in home health, geriatrics, hospice, and primary care
- Geriatric training/experience preferred
