About the role
- Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and determine physical, mental, and social needs
- Take patient history, perform examinations, and determine diagnoses
- Provide written documentation of patient visit per NCQA standards
- Take patient vital signs as necessary
- Place case management referrals and communicate with primary care providers as necessary
- Communicate with patients, caregivers, agency nurses, other providers and vendors to assure proper diagnosis and care coordination
- Perform all clinical duties while observing OSHA Universal Precautions
- Maintain patient confidentiality
- Attend required meetings and in-services and participate in committees as requested
- Participate in professional development activities and maintain professional licenses and affiliations
- Provide care via in-home visits and telehealth as delegated by the Collaborative Physician
- Collaborate with teammates, physicians, medical staff, patients, caregivers, agency nurses, providers and vendors
Requirements
- Must reside within Hillsborough County, Florida
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification in ANCC, AANP, or NCCPA
- Active CPR Certification
- Current enrollment in Medicare/Medicaid
- Must maintain a valid driver’s license and good driving record
- Outstanding EHR skills
- Geriatric training/experience (preferred)
- Ability to conduct comprehensive in-home assessments, take vitals, and determine diagnoses
- Ability to place case management referrals and communicate with PCPs
- Ability to communicate with patients, caregivers, agency nurses, providers, and vendors
- Maintain patient confidentiality and professional licenses/affiliations
- Adaptability to differing weather conditions and patients’ home/residential environments
- Full range of body motion including handling/lifting patients; manual and finger dexterity; normal visual acuity and hearing; standing, bending, walking and stair climbing
- Regular lifting/carrying items weighing up to 50 pounds
- Ability to ride in automobile or van up to 150 miles daily in urban and/or rural settings
- Ability to drive, if necessary
