Community Manager

• Enroll eligible patients into the HarmonyCares program via outreach
• Collaborate with Community Account Executive to outreach physician offices and healthcare facilities
• Conduct telephonic outreach to eligible patients and caregivers to explain and enroll patients into the HarmonyCares program
• Conduct telephonic outreach to physician offices, hospitals and skilled nursing facilities to help engage their patients
• Understand and explain HarmonyCares program to patients, caregivers and providers and handle objections
• Build relationships with community providers and facilities and collaborate with cross-functional team to ensure great patient experience
• Leverage electronic tools, spreadsheets and reports to research, identify approaches, document and present plans
• Meet or exceed established monthly engagement targets
