About the role
- Enroll eligible patients into the HarmonyCares program via outreach
- Collaborate with Community Account Executive to outreach physician offices and healthcare facilities
- Conduct telephonic outreach to eligible patients and caregivers to explain and enroll patients into the HarmonyCares program
- Conduct telephonic outreach to physician offices, hospitals and skilled nursing facilities to help engage their patients
- Understand and explain HarmonyCares program to patients, caregivers and providers and handle objections
- Build relationships with community providers and facilities and collaborate with cross-functional team to ensure great patient experience
- Leverage electronic tools, spreadsheets and reports to research, identify approaches, document and present plans
- Meet or exceed established monthly engagement targets
Requirements
- Associates degree, or 3+ years of progressive career growth
- 2+ years of related experience in sales, patient engagement, community outreach or customer service, preferably in the medical/healthcare industry
- Proficiency in Microsoft Excel, Word, and PowerPoint
- An engaging personality with great active listening, written and verbal communication skills
- Detail oriented with excellent documentation skills
- Organized and responsive with great follow-up
- Compassion, interest in people and professional presentation and demeanor
- Curious and continuous learner, passion for solving problems
- Bilingual in Spanish, Mandarin or other language (preferred)
- Contact center experience (preferred)
- Comfort with CRM and EMR tools (preferred)
