About the role

Key responsibilities & impact

Support successful patient transitions from hospital to home, chronic disease management, care coordination, and patient triage
Coordinate deployment of home-based care services with RPM team members
Access, navigate, and document in the EMR System
Maintain patient privacy per HIPAA guidelines
Act as a point of contact for patients enrolled in the Chronic Care Management program
Monitor alerts and check for missing data in a timely manner
Assist with remote monitoring equipment setup and troubleshooting
Receive and relay patient questions/concerns and coordinate care per established protocols
Schedule follow-up appointments for patients as needed
Collaborate with Primary Care, Care Coordination, EMS crews, and other specialists to meet patient needs

Requirements

What you’ll need

High School education or GED required
Prior clinical or direct patient care experience preferred but not required
Proficiency with Microsoft programs (Excel, Word, Outlook, Teams, etc.) preferred
Professional communication and teamwork required
