Pharmacist, Population Health – Value Based Reimbursement

About the role
Key responsibilities & impact- Responsible for improving the health of populations through optimized medication management.
- Analyze data from various sources to identify medication-related trends and areas for improvement.
- Develop and implement strategies to optimize medication use and improve quality metrics.
- Support value-based care initiatives.
- Assess patient’s medication therapy for indications, effectiveness, safety, and adherence issues.
- Build relationships with providers to create a feedback loop between pharmacy and clinical teams.
- Ensure documentation of encounters in the electronic health record.
Requirements
What you’ll need- Graduate of an ACPE-accredited School of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree.
- Current Licensure as a pharmacist in the state of New York and in the state of Pennsylvania.
- Completion of a pharmacy residency program (PGY1) or equivalent clinical experience.
- Board certification in ambulatory care pharmacy (BCACP) or other relevant certification preferred.
- Strong clinical knowledge and experience in medication therapy management.
- Proficiency in using electronic health records (EHR) and other healthcare technology.
