About the role

Key responsibilities & impact
  • Respond promptly to after-hours calls from patients, families, and caregivers.
  • Conduct patient assessments to evaluate pain, symptom management, and overall comfort.
  • Administer medications, perform wound care, and assist with medical procedures as necessary.
  • Provide emotional support and education to patients and their families regarding end-of-life care.
  • Collaborate closely with the interdisciplinary hospice team, including physicians, social workers, chaplains, and volunteers.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Maintain accurate and thorough patient records in compliance with hospice regulations, the organization's policy, and best practices.
  • Ensure the delivery of compassionate and culturally sensitive care tailored to each patient's unique needs.
  • Participate in on-call rotation schedules, including weekends and holidays, to ensure 24/7 availability for patients.

Requirements

What you’ll need
  • Current Registered Nurse (RN) license in the State of Georgia and current CPR certification.
  • Minimum of three (3) years of nursing experience, with previous hospice or palliative care experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Compassionate and empathetic demeanor.
  • Ability to work independently and as part of a collaborative team.
  • Reliable transportation and a valid driver's license.
  • Willingness to work on-call shifts, including nights, weekends, and holidays.
