About the role
- Support patients through telephonic outreach, care coordination, and ongoing health education.
- Monitor care plans, schedule appointments, review medical records, document in EHRs, and facilitate access to medical services.
- Manage and support patients’ healthcare needs via telephonic outreach.
- Conduct and document visits by using several technology platforms and EHRs.
- Monitor adherence to care plans, evaluate effectiveness, monitor patient progress, and facilitate changes as needed.
- Facilitate patient access to appropriate medical and specialty providers.
- Educate patients and family/caregiver(s) about relevant community resources.
- Coordinate continuity of patient care with external healthcare organizations and facilities.
- Support patient self-management of disease and behavior modification interventions.
- Provide patient health counseling, education, and instruction.
Requirements
- CMA Certification
- Maintaining a Certification is an ongoing requirement of this role
- Minimum of 1 year work experience in a healthcare setting involving patients with complex chronic disease states preferred.
- Chronic Care Management experience required. (CCM, PCM, APCM)
- Proficiency with a personal computer
- Technical and analytical skill set
- Excellent communication, organizational, and interpersonal skills are necessary
- Experience with Microsoft Excel preferred.
