About the role
- Provide neurobehavioral health services to residents in long-term care facilities
- Provide evaluation
- Provide condition-specific support therapy, or when indicated, insight-based therapy
- Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues
- Engage with the family as indicated
- Partner with specialty clinicians to provide holistic, continuity of care for elderly individuals with neurocognitive disorders and other neurologic and/or psychiatric conditions
Requirements
- Ability to work as part of an interdisciplinary treatment team
- Good time management skills
- Able to function autonomously with regional Area Manager support
- Master’s degree in Counseling or in Social Work
- Currently licensed as an LPCC or LISW in the state in which you are applying
- Passion for geriatrics (preferred)
- Experience in inpatient settings such as extended care facilities (preferred)
- Experience with brief bedside assessment screens (cognitive, mood, etc.) (preferred)
- Motivated by productivity-based positions (preferred)
- Ability to navigate and document on an Electronic Medical Record (EMR) system (preferred)
- All GuideStar employees must be vaccinated against the flu
- Drug testing will be administered to all final candidates for whom an offer has been extended
