About the role

Key responsibilities & impact

Build trusting, ongoing relationships with patients, families, caregivers, and healthcare providers
Engage high-risk and targeted patient populations through calls, texts, video visits, portals, emails, and AI agents
Identify medical, behavioral, social, emotional, and financial needs
Help patients understand and follow care plans, health goals, and preventive care needs
Conduct nonclinical assessments, surveys, and interventions
Recognize and escalate patient-condition changes, adherence risks, and clinical concerns
Prioritize interventions using clinical insight and support population-health strategies
Coordinate referrals, transportation, appointments, specialty care, and community resources
Escalate medical issues to RN Care Managers, Social Workers, provider offices, or Guidehealth leadership
Support eCQM, HEDIS, and commercial payer quality measures
Support risk adjustment and accurate documentation of patient conditions
Close care gaps and reduce avoidable emergency visits, hospitalizations, and readmissions
Document interactions, assessments, and interventions in EHR and Guidehealth systems
Manage referrals, track tasks, complete reconciliations, and maintain records
Support the design and implementation of AI-assisted outreach

Requirements

What you’ll need

Fully bilingual in Arabic and English, including full medical terminology
2–3 years of related experience in patient navigation, medical assistance, health promotion, care coordination, or similar roles
Valid Medical Assistant Certification
Equivalent experience may include several years in similar roles without formal certification
Experience working directly with patients in clinical or non-clinical settings, preferably with complex medical or social needs
Demonstrated experience with EHR documentation and remote-work effectiveness
Familiarity with Epic, Athena, Cerner, analytics tools, and Microsoft Office
Ability to follow Guidehealth policies and procedures
Ability to provide own internet connection with minimum 100 Mbps download and 10 Mbps upload
Ability to work during stated working hours and participate in virtual meetings with camera on
