About the role

Key responsibilities & impact

Work with individuals with developmental disabilities and their families to advocate for needed services
Participate in assessment of individuals' needs
Develop, implement, and coordinate Individual Program Plans
Manage a wide range of active cases as a social worker
Gather information for interdisciplinary team use
Design and monitor program plans while analyzing data
Ensure appropriate and effective service delivery to individuals
Participate in outreach services and committees as assigned

Requirements

What you’ll need

Knowledge of principles and techniques of counseling individuals
Knowledge of normal and abnormal individual and group behavior, human growth and development of family relationships
Knowledge of community structure and organization
Ability to develop skills for independent work and exercise judgment
Excellent communication skills in producing business documents
Ability to stimulate cooperation of clients and community agencies
Ability to establish and maintain effective working relationships
Bachelor’s Degree from accredited school of social welfare or related human services field
Fluency in Cantonese
Case Management experience is strongly desired
Reliable transportation for program visits
