About the role
Key responsibilities & impact- Responsible for providing outpatient care and coordinating services across the care continuum for defined patient populations.
- Includes risk assessment, transitional care planning, psychosocial and functional status assessment, patient advocacy, motivational interviewing, education of the patient/family and monitoring quality indicators.
- Works collaboratively with the primary care team to meet patient’s needs.
- Assists patients to safely and effectively navigate through the care continuum.
- Performs concurrent care review in accordance with approved clinic and quality guidelines.
Requirements
What you’ll need- Master’s degree is required.
- Experience in motivational interviewing / health coaching / goal setting is required.
- Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) licensure is required.
- All licensure must be current and active for the state of Georgia.
- Must have current BLS certification from American Heart Association.
