About the role

Key responsibilities & impact
  • Frequent travel to patients’ homes to support their engagement with our virtual clinic, on average 3–4 days of travel per week
  • Drive long distances if needed, including travel that may require an overnight stay when travel exceeds 4 hours of one-way driving
  • Provide coverage for Mental Health Guides as needed
  • Establish and maintain trusted relationships with Flourish Health patients/families utilizing a patient centered, strengths-based, and trauma-responsive approach
  • Adapt to changing workflows as the clinic grows
  • Be a steadfast, caring, and non-judgmental presence for patients/families
  • Co-develop and support adherence to individualized care/engagement plans
  • Utilize app-based messaging and phone calls as primary means of engagement
  • Identify needs and provide mental health advocacy for patients in various areas
  • Act as a conduit to a patient’s full care team; proactively identify needs for additional support and/or resources
  • Maintain a working vehicle in order to provide quality patient care through in-home visits.

Requirements

What you’ll need
  • Associate’s, Bachelor’s or Master’s degrees
  • 3-5 years of experience in a relevant field required
  • Demonstrated experience or interest working with people with serious mental illness
  • Experience working with adolescents, young adults, and/or families
  • Working with underserved communities with complex family situations
  • Bilingual, Spanish and English speaking abilities (bonus)
  • Reliable motor vehicle, active driver’s license, and valid auto insurance for in-person visits.
  • Must be 18 years of age or older.
