About the role

Key responsibilities & impact
  • Deliver professional outpatient pharmacy services and direct patient care in accordance with recognized medical standards and CSU Medical Staff Bylaws.
  • Accurately document all care rendered using the Electronic Health Record (EHR) system, ensuring records meet Joint Commission and CMS standards for timeliness, legibility, and accuracy.
  • Collaborate with multidisciplinary teams, including Community Health Nurses and Social Workers, to coordinate patient management for medical and social needs.
  • Coordinate and integrate clinical information into the ongoing Health Care Program at the Service Unit and ensure timely follow-up care.
  • Participate in quality monitors and performance improvement efforts as assigned.

Requirements

What you’ll need
  • Completion of an accredited Pharmacy program.
  • Minimum of six (6) months of experience in the specialty.
  • No lapse in services of more than six consecutive months within the six months prior to beginning the assignment.
  • Demonstrated respect for American Indian and Alaska Native practices and traditions.
  • Ability to read, understand, speak, and write English effectively to communicate with patients and staff.
  • Basic knowledge and skills required to navigate computer systems and electronic health record software.
  • Adherence to the Federal Code of Conduct and a high standard of ethical behavior.
