About the role

Key responsibilities & impact

Deliver professional outpatient pharmacy services and direct patient care in accordance with recognized medical standards and CSU Medical Staff Bylaws.
Accurately document all care rendered using the Electronic Health Record (EHR) system, ensuring records meet Joint Commission and CMS standards for timeliness, legibility, and accuracy.
Collaborate with multidisciplinary teams, including Community Health Nurses and Social Workers, to coordinate patient management for medical and social needs.
Coordinate and integrate clinical information into the ongoing Health Care Program at the Service Unit and ensure timely follow-up care.
Participate in quality monitors and performance improvement efforts as assigned.

Requirements

What you’ll need

Completion of an accredited Pharmacy program.
Minimum of six (6) months of experience in the specialty.
No lapse in services of more than six consecutive months within the six months prior to beginning the assignment.
Demonstrated respect for American Indian and Alaska Native practices and traditions.
Ability to read, understand, speak, and write English effectively to communicate with patients and staff.
Basic knowledge and skills required to navigate computer systems and electronic health record software.
Adherence to the Federal Code of Conduct and a high standard of ethical behavior.
