About the role
Key responsibilities & impact- Join the Social Work Hospital Discharge Team (West).
- Work collaboratively with social workers contributing to personal outcome focused assessments as part of discharge planning.
- Engage with individuals, families and carers to address some of the practical barriers to discharge whilst promoting their rights and strengths to achieve better outcomes.
- 36hrs per week Monday to Friday on a permanent basis with core hours of 8am – 4pm.
Requirements
What you’ll need- Possess active listening skill, demonstrate empathy, clarity and be able to understand and be understood.
- Have awareness of the assessment process including assessment of risk and risk management
- Possess good report writing skills
- Be able to work effectively with a range of professionals
- Have the ability to remain calm and professional under pressure, be able to work independently as well as part of a team.
- Have experience of working in a relevant setting. (multidisciplinary working is desirable)
- Have an awareness of personal centred practice and practice in a way which empowers individuals.
- Ability to think creatively and have excellent problem-solving skills
- Have an ability to establish and maintain a professional relationship with service users
- Have the ability to travel throughout Fife.
- Have good IT skills
