About the role

Key responsibilities & impact
  • Join the Social Work Hospital Discharge Team (West).
  • Work collaboratively with social workers contributing to personal outcome focused assessments as part of discharge planning.
  • Engage with individuals, families and carers to address some of the practical barriers to discharge whilst promoting their rights and strengths to achieve better outcomes.
  • 36hrs per week Monday to Friday on a permanent basis with core hours of 8am – 4pm.

Requirements

What you’ll need
  • Possess active listening skill, demonstrate empathy, clarity and be able to understand and be understood.
  • Have awareness of the assessment process including assessment of risk and risk management
  • Possess good report writing skills
  • Be able to work effectively with a range of professionals
  • Have the ability to remain calm and professional under pressure, be able to work independently as well as part of a team.
  • Have experience of working in a relevant setting. (multidisciplinary working is desirable)
  • Have an awareness of personal centred practice and practice in a way which empowers individuals.
  • Ability to think creatively and have excellent problem-solving skills
  • Have an ability to establish and maintain a professional relationship with service users
  • Have the ability to travel throughout Fife.
  • Have good IT skills
