About the role

Key responsibilities & impact

Looking for a dynamic, enthusiastic and motivated Social Worker to join the Social Work Hospital Discharge Team (West).
Work with adults who have physical and/or mental health needs, their families and carers promoting a person-centred approach to discharge planning.
Work as part of a multidisciplinary team undertaking personal outcome focused assessments to support adults to achieve safe and timely discharges.

Requirements

What you’ll need

Undertake person outcome focused assessments for adults who are in hospital to achieve timely discharges in line with legislation and policy
Complete comprehensive risk assessments and risk management plans
Work collaboratively with a range of professionals
Have experience in adult social work or a health-related setting. Experience of hospital social work is desirable
Be able to manage complex cases and work under pressure
Possess good report writing skills
Have the ability to travel throughout Fife.
Have good IT skills
