About the role
Key responsibilities & impact- Looking for a dynamic, enthusiastic and motivated Social Worker to join the Social Work Hospital Discharge Team (West).
- Work with adults who have physical and/or mental health needs, their families and carers promoting a person-centred approach to discharge planning.
- Work as part of a multidisciplinary team undertaking personal outcome focused assessments to support adults to achieve safe and timely discharges.
Requirements
What you’ll need- Undertake person outcome focused assessments for adults who are in hospital to achieve timely discharges in line with legislation and policy
- Complete comprehensive risk assessments and risk management plans
- Work collaboratively with a range of professionals
- Have experience in adult social work or a health-related setting. Experience of hospital social work is desirable
- Be able to manage complex cases and work under pressure
- Possess good report writing skills
- Have the ability to travel throughout Fife.
- Have good IT skills
