About the role

  • The Case Manager will engage with clients who have a primary history of mental illness.
  • Develop strengths assessment with clients.
  • Offer crisis and safety planning, in collaboration with mental health providers.
  • Create individualized planning with the clients in the areas of health, education, mental health, vocation, socialization, benefits, legal and life skills.
  • Meet a minimum of 2 times per month to support the youth with their goal achievements.
  • The Case Manager will assist in the coordination of services with providers, benefit systems and the community.
  • Assist in the linkage to service providers, benefit systems and the community.
  • Advocate for clients as needed.
  • Help clients navigate service systems so they get the support that they need in their recovery.
  • The Case Manager will initiate crisis intervention with linkage to appropriate mental health or health services, as necessary.
  • Report all client incidents and accidents.
  • Inform supervisor when other providers are not collaborating in the effort to achieve services for the client or have plans to inappropriately discharge a client from housing.
  • Follow guidelines set by the Office of Mental Health and Division of Mental Hygiene SPOA unit Risk Management.
  • Attend and participate in monthly trainings for the evidence based High Fidelity Model.
  • All other duties as assigned.

Requirements

  • Bachelor’s Degree in Social Work with 2 years of related experience or Master’s Degree with 1 year of related experience required.
  • Excellent interpersonal and organizational skills required.
  • Strong verbal and written communication skills required.
  • Ability to solve problems and work as part of a team required.
  • Proficient computer skills, including Microsoft Office, required.
  • Valid and clean NYS Driver’s License required.
