About the role

  • The Case Manager will conduct intakes, assessments, service planning and follow up.
  • Provide access and help navigate systems and programs.
  • Obtain all proper consent forms to coordinate care.
  • Work with government and community agencies.
  • Ensure clients receive needed services, including provision of transportation to appointments.
  • The Case Manager will conduct program outreach.
  • All other duties as assigned.

Requirements

  • Associates Degree required.
  • Bachelor's Degree in Human Services, Social Work, Sociology or related field required.
  • At least five years’ experience assessing and linking adults to needed services and programs required.
  • Excellent interpersonal and verbal and written communication skills required.
  • Proficient computer skills, including Microsoft Office required.
  • Bilingual in Spanish preferred.
