About the role
- The Case Manager will conduct intakes, assessments, service planning and follow up.
- Provide access and help navigate systems and programs.
- Obtain all proper consent forms to coordinate care.
- Work with government and community agencies.
- Ensure clients receive needed services, including provision of transportation to appointments.
- The Case Manager will conduct program outreach.
- All other duties as assigned.
Requirements
- Associates Degree required.
- Bachelor's Degree in Human Services, Social Work, Sociology or related field required.
- At least five years’ experience assessing and linking adults to needed services and programs required.
- Excellent interpersonal and verbal and written communication skills required.
- Proficient computer skills, including Microsoft Office required.
- Bilingual in Spanish preferred.
