About the role

  • Operates as a behavioral health consultant on the medical team utilizing the Primary Care Behavioral Health (PCBH) model of care.
  • Sees patients of all ages with wide-ranging medical, behavioral health, and behaviorally influenced conditions.
  • Provides episodic behavioral health services and interventions to a high volume of patients.
  • Facilitates group therapy sessions.
  • Designs group therapy curriculum based on relevant clinical topics to support patient needs.
  • Conducts culturally sensitive, strength-based assessments and short-term therapy for clients of all ages and families.
  • Prioritizes and provides on-demand consultations as well as manages scheduled appointments to individuals, couples, and/or families.
  • Utilizes therapeutic modalities that support wellness, improved quality of life, and behavior change.
  • Consults with medical colleagues in real-time to enhance understanding of the patient and provide decision support for treatment planning.
  • Closes all charts within forty-eight (48) hours of visit; including assessments, progress notes, and billing.
  • Assists in staff and provider training in integrated behavioral health-primary care model.
  • Participates in behavioral health quality improvement activities.
  • Participates in weekly group supervision and seeks peer consultation as needed.
  • Provides risk assessments and develops comprehensive safety plans.

Requirements

  • Master’s Degree in Social Work, Psychology, or Mental Health Counseling from a Massachusetts accredited, licensure-eligible program required
  • If not independently licensed, candidate must be actively working towards independent licensure within 6 months of start date.
  • Professional Licensure/Special Skills and Certification preferred: LICSW, LMHC, or PsyD. preferred
  • Advanced experience in leading groups in a behavioral health setting required
  • Minimum of 2 years direct clinical work with individuals, children, and families is required.
