About the role
- Assess client needs, develop and implement comprehensive care plans.
- Provide therapeutic interventions and functional life skill development in various settings, including homes, schools, and the community.
- Serve as the team facilitator, coordinating and facilitating interdisciplinary meetings.
- Demonstrate effective integration of services to support client recovery and overall well-being.
- Conduct comprehensive assessments to evaluate client mental health, emotional well-being, and social functioning.
- Develop individualized care plans in collaboration with clients to address their specific needs and goals.
- Provide direct behavioral support, evidence-based interventions, and life skill development to support clients with achieving their goals.
- Engage clients in meaningful activities and social interactions to promote mental and emotional well-being.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field preferred.
- Direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, behavioral health facilities, and/or related community-based organizations.
- One (1) year of direct experience in a Behavioral Health or Social Services setting such as outpatient clinics, behavioral health facilities, and/or related community-based organizations, preferred.
- Four (4) years full-time equivalent experience in a Behavioral Health setting as a specialist in the fields of physical restoration, social adjustment, or vocational adjustment for Social Worker II.
- Master's degree in social work, Psychology or related field required for Social Worker III and IV.
- Current active and unrestricted California license as a Licensed Clinical Social Worker, Licensed Marriage Family Therapist, Licensed Professional Clinical Counselor, or Psychologist for Social Worker IV.
