About the role
Key responsibilities & impact- As a home care LNA, you will provide peaceful, dedicated personal care and clinical support to our clients in the comfort of their homes.
- Engage clients in meaningful conversation, puzzles, walks, and activities to keep them socially active and mentally sharp.
- Provide hands-on support with Activities of Daily Living (ADLs) including bathing, grooming, dressing, skin care, and personal hygiene.
- Assist clients with ambulation, transfers, and positioning using proper body mechanics and assistive devices.
- Assist with meal planning and nutritional cooking according to dietary restrictions; monitor and encourage hydration.
- Provide timely reminders for clients to take prescribed medications and support routine adherence.
- Maintain a safe, clean, and hygienic environment by handling laundry and light cleaning.
- Engage clients in meaningful social interaction and accompany or drive them to local medical appointments and essential outings.
- Closely observe, document, and report any changes in the client’s physical or mental status to the nursing supervisor.
Requirements
What you’ll need- Active New Hampshire LNA (Licensed Nursing Assistant) certificate in good standing (Required).
- 1+ years of experience as an LNA in home health, a facility, or a hospital setting preferred.
- Valid Driver’s License, active auto insurance, and a reliable vehicle for traveling to client homes (Required).
- Current certification in CPR and First Aid (Preferred).
- Strong communication skills and a reliable work ethic.
