About the role

Key responsibilities & impact
  • Works with a multi-disciplinary team including mental health therapist, youth or family peer specialist, therapeutic recreation specialists, and education and healthcare professionals to support families.
  • Provides direction in the development and implementation of cross-system care plans.
  • Engages families to help avoid out-of-home placement and improve safety and satisfaction.
  • Participates in on-call rotation for crisis services.
  • Develops community resource programs to achieve desired outcomes for children and families.
  • Promotes WISe programs throughout the county and identifies coordination opportunities.
  • Recognizes barriers in care systems and implements practices to sustain improvements.
  • Develops goals and objectives for WISe program services across systems of care.
  • Communicates effectively with child and family serving agencies and serves as liaison.
  • Coordinates cross-agency communication and planning as needed.
  • Transports clients/families to program activities using company vehicles.
  • Attends and participates in internal and external meetings on behalf of Excelsior.

Requirements

What you’ll need
  • High School Diploma required, BA in counseling or related field preferred.
  • Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
  • Comprehensive knowledge of community resources and public systems.
  • Minimum three years of working with youth and families preferred.
  • Exceptional communication and interpersonal skills.
  • Community Health Worker certification preferred.
  • Strong multitasking and organizational skills.
  • Ability to work in a fast-paced environment and make ethical and safe decisions.
  • Ability to manage individual stress and perform essential job functions in a safe and professional manner.
  • Must be able to pass a criminal background check.
  • Must be 21 years or older.
  • Valid state driver’s license.
