• Ability to Create a Plan of Care by evaluation and assessment of patient’s condition and maintain continuity of and or multidisciplinary services whenever needed to ensure good coordination of care.
• Timely Documentation according to state, federal and organizational guidelines.
• Participation in Case Conference meetings, and In-Service Reviews.
