About the role

Key responsibilities & impact

Conduct telehealth visits for patients enrolled in the House Calls program
Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
Perform care management needs assessments, follow-ups, and care plan adjustments
Review patient histories, labs, imaging, and diagnostic results
Support medication management and reconciliation
Coordinate care with primary care providers, specialists, and care management teams
Document visits and clinical decisions accurately in the EMR
Ensure compliance with clinical protocols, quality standards, and regulatory requirements
Participate in interdisciplinary case reviews and quality improvement initiatives
Provide virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes

Requirements

What you’ll need

Active Nurse Practitioner (NP) license in the required state(s)
National NP certification (ANCC or AANP)
Minimum 2+ years of clinical experience as a Nurse Practitioner
Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred
Prior telehealth or remote care experience preferred
Strong clinical judgment and comfort managing complex patients remotely
Excellent communication and documentation skills
Must be licensed in NY and one of the other states listed or willing to obtain
Proficiency with EMR/EHR systems
Comfortable using telehealth platforms and remote monitoring tools
Active DEA registration (or willingness to obtain, if applicable)
