About the role

Key responsibilities & impact
  • Deliver person-centered care in homes and community settings
  • Assist in creating and continuously revising individualized service plans
  • Monitor and ensure effective implementation of service plans
  • Provide resource linkage and case management support
  • Submit required documentation within established timeframes
  • Maintain communication with individuals and key stakeholders for coordinated planning and service delivery

  • Travel to client homes and community locations
  • Engage with clients for extended periods in field-based settings

Requirements

What you’ll need
  • Registered as a QMHP or QMHP-T with the Virginia Board of Counseling, or certified as a QPPMH
  • Bachelor’s degree or higher in a Human Services-related field, such as Psychology, Social Work, or Counseling
  • At least 1 year of behavioral health experience
  • Valid driver’s license and reliable transportation
  • Current CPR/First Aid certification
  • Current TB screening
  • Ability to travel to client homes and community locations
  • Ability to sit, stand, and engage with clients for extended periods
  • Availability for flexible scheduling, including evenings or weekends when required
