About the role
- Provide case management services to individuals with serious mental illness (SMI)
- Conduct regular outreach to assigned members to build rapport and assess needs
- Link members to needed services/resources and provide short-term support (up to 6 months)
- Respond to complex cases and account specific requests
- Conduct assessments to identify individual needs and develop care plans
- Monitor and evaluate effectiveness of care plans and modify as needed
- Coordinate with internal and external resources to meet identified needs
Requirements
- Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing
- Minimum of 3 years of clinical experience in social work counseling
- Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist
- Previous experience in case management and telephonic and/or in person coaching
- Experience with complex psychiatric and substance abuse disorders
- Knowledge of behavioral health systems of care in Virginia strongly preferred.
