About the role
- Perform case management telephonically and/or by home visits within scope of licensure for members with behavioral health and substance use disorder needs
- Serve as subject matter expert in targeted clinical areas (e.g., Eating Disorders, Maternity, Alcohol/Drug, Autism Spectrum Disorders)
- Respond to complex cases and account-specific requests
- Use screening criteria and clinical judgment to assess member needs
- Conduct assessments and develop individualized care plans; monitor and modify plans as needed
- Support member access to appropriate, quality and cost-effective care
- Coordinate with internal and external resources and collaborate with providers
- Serve as a resource to other behavioral health case managers and participate in cross-functional teams, projects, and initiatives
Requirements
- MA/MS in social work, counseling, related behavioral health field, or degree in nursing (or equivalent combination of education and experience)
- Minimum of 3 years clinical experience in social work/counseling with a broad range of complex psychiatric and substance abuse disorder treatment
- Current active unrestricted license (RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW) or Clinical Psychologist as allowed by applicable state laws
- Previous experience in case management and telephonic and/or in-person coaching with members with complex psychiatric/substance abuse and/or medical disorders
- Managed care experience required
- For Government business: LAPC or LAMFT acceptable where allowed
- Some field-based work required; ability to perform home visits
- Must be within reasonable commuting distance to posting location or have an accommodation as required by law
- Preferred: experience in health coaching and motivational interviewing techniques
- For Puerto Rico associates: current PR health certificate and PR Law 300 certificate required (if applicable)
- May require COVID-19 and Influenza vaccination for certain patient/member-facing roles
