About the role
Key responsibilities & impact- Verify the Plan of Care before each visit
- Provide care according to RN delegation, physician orders, assessment data, and established standards and guidelines
- Incorporate patient care goals into nursing visits and document them in skilled nursing notes
- Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other involved personnel
- Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
- Implement patient safety plans using patient, family, and community resources
- Teach patients appropriate self-care techniques while delivering quality care
- Suggest revisions to the Plan of Care and notify the RN Case Manager
- Provide substantial and specialized nursing services under the direction of a Registered Nurse or physician
- Collaborate with the RN Case Manager during normal business hours
- Document medication regimens and update medication profiles
- Treat patients and caregivers in the highest and most effective manner
- Take actions to prevent avoidable hospitalizations and ER visits
- Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel
- Report to the Clinical Team Manager
Requirements
What you’ll need- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting; hospice preferred
- Experience in a home health environment preferred
- 50% travel required
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift 50–100 lbs.
- Valid driver’s license and insurance, or reliable transportation to travel between patient homes and assigned work locations
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation
- E-Verify employment authorization process after offer acceptance and Form I-9 completion
