About the role

Key responsibilities & impact
  • Ensure activities align with Elara Caring’s vision and leadership direction
  • Maintain patient and staff privacy and confidentiality under HIPAA Privacy Final Rule
  • Assess assigned home health patient and family caseloads for physical, psychosocial, and environmental needs
  • Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating treatment plans
  • Gather physical, psychological, social, and spiritual patient data and incorporate it into care plans
  • Perform initial evaluations and re-evaluate nursing needs during each visit
  • Conduct follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, health-condition changes, environmental concerns, and unsafe facility conditions to relevant clinical and care personnel
  • Revise care plans in response to patient-care issues and notify the Clinical Supervisor, physician, facility, and team members
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year of experience in a clinical care setting as a nurse
  • Ability and willingness to travel within the branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Dependable vehicle
  • Valid driver’s license
  • Current auto insurance in accordance with state laws
  • Ability and willingness to travel 50%
  • Associate’s Degree or Bachelor’s Degree in Nursing is preferred
  • Previous Home Health or Hospice experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
  • Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
