About the role
Key responsibilities & impact- Evaluate patients and develop individualized treatment plans
- Provide physical therapy services in the home setting
- Monitor patient progress and adjust care plans as needed
- Collaborate with physicians, caregivers, and the interdisciplinary care team
- Complete timely and accurate clinical documentation
- Supervise therapy assistants and support staff, as appropriate
- Travel between patient homes and assigned work locations
Requirements
What you’ll need- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
