About the role
Key responsibilities & impact- Align all activities with Elara Caring’s vision and the leadership of the Hospice team
- Prevent unwanted hospitalizations by meeting patient and family needs in the home
- Prepare and maintain accurate patient records, charts, and documents
- Comply with applicable laws and regulations regarding Collaborative Agreements
- Complete accurate billing and coding for patient encounters according to CMS guidelines
- Initiate reimbursement for medically necessary visits
- Provide training and continuing education for staff
- Assist in developing clinical practice guidelines and standards supporting quality palliative care
- Maintain confidentiality regarding patients, families, staff, and the organization
- Collaborate and communicate with the Medical Director, attending physician, branch leadership, and IDG
- Perform other assigned duties and projects
- Travel up to 50% as needed to support business and patient needs
- Report to the Branch Director
Requirements
What you’ll need- Master’s Degree in Nursing
- Certification through a national Nurse Practitioner Licensing board
- 3+ years of clinical nursing experience
- 1+ year of post-graduate clinical nursing experience in hospice and palliative care
- Current licensure in the state of practice as a Nurse Practitioner
- Maintain Nurse Practitioner licensure according to applicable laws and regulations
- Active DEA license
- Able and willing to travel up to 50% as needed
- Prior experience in Family Medicine, Geriatrics, and psychiatry preferred
- Board Certification in Palliative Care preferred
