About the role
Key responsibilities & impact- Ensure all activities align with Elara Caring’s vision and Hospice team leadership
- Assess hospice patients and families to identify physical, psychosocial, environmental, psychological, social, and spiritual needs
- Review documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations
- Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other care personnel
- Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care
- Teach hospice philosophy and pain and symptom management to staff in alternate care settings
- Provide appropriate support at time of death and perform bereavement assessments
- Report to the Clinical Team Manager
- Travel 50% as required for patient care and job tasks
Requirements
What you’ll need- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current state license as a Registered Nurse (RN)
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver’s license and insurance
- Reliable transportation to perform job tasks
