About the role

Key responsibilities & impact
  • Assess assigned behavioral health patient and family caseloads to identify physical, psychosocial, and environmental needs
  • Assume primary responsibility for caseloads by assessing, planning, coordinating, implementing, and evaluating treatment plans
  • Assess patient needs and collect physical, psychological, social, and spiritual health data for care planning
  • Conduct initial evaluations and reassess nursing needs during each visit
  • Make follow-up visits based on patient, family, and facility needs
  • Communicate significant findings, problems, and health changes to supervisors, physicians, facilities, and care personnel
  • Revise care plans in response to patient-care issues and notify relevant clinical and facility personnel
  • Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care
  • Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
  • Initiate, supervise, and document behavioral health aide and LPN care delivery
  • Report unusual or potentially problematic nursing facility issues
  • Provide support at time of death and perform bereavement assessments
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • High School Diploma or GED
  • Current, unrestricted state RN license
  • 1+ years of experience in a clinical care setting as a nurse
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Able and willing to travel within the branch/office coverage area
  • Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Associates Degree or Bachelors Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with Homecare HomeBase is preferred
