About the role
Key responsibilities & impact- Assess assigned behavioral health patient and family caseloads to identify physical, psychosocial, and environmental needs
- Assume primary responsibility for caseloads by assessing, planning, coordinating, implementing, and evaluating treatment plans
- Assess patient needs and collect physical, psychological, social, and spiritual health data for care planning
- Conduct initial evaluations and reassess nursing needs during each visit
- Make follow-up visits based on patient, family, and facility needs
- Communicate significant findings, problems, and health changes to supervisors, physicians, facilities, and care personnel
- Revise care plans in response to patient-care issues and notify relevant clinical and facility personnel
- Counsel and instruct patients, facilities, and families in following the Interdisciplinary Plan of Care
- Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
- Initiate, supervise, and document behavioral health aide and LPN care delivery
- Report unusual or potentially problematic nursing facility issues
- Provide support at time of death and perform bereavement assessments
- Report to the Clinical Manager or Clinical Supervisor
Requirements
What you’ll need- High School Diploma or GED
- Current, unrestricted state RN license
- 1+ years of experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Able and willing to travel within the branch/office coverage area
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Associates Degree or Bachelors Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with Homecare HomeBase is preferred
