About the role
Key responsibilities & impact- Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment data, and established standards
- Incorporate patient care goals into nursing visits and document care on skilled nursing notes
- Communicate significant findings, problems, and patient or environmental changes to the RN Case Manager, physician, clinical supervisor, and involved personnel
- Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
- Implement patient safety plans using patient, family, and community resources
- Teach patients appropriate self-care techniques while delivering quality care
- Suggest revisions to the Plan of Care and notify the RN Case Manager
- Provide specialized nursing services under the direction of a Registered Nurse or physician
- Collaborate with the RN Case Manager during business hours
- Document medication regimens and update medication profiles
- Treat patients and caregivers effectively and respectfully
- Take actions to prevent avoidable hospitalizations and ER visits
- Provide supportive communication to physicians, patients, families, visitors, and agency personnel
- Report to the Clinical Team Manager
Requirements
What you’ll need- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current state license as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Hospice or home health experience preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Valid driver's license and insurance
- Reliable transportation to perform job tasks
