About the role
Key responsibilities & impact- Provide high-quality home health care to patients in their preferred home setting
- Assess assigned home health patient caseloads and identify physical, psychosocial, and environmental needs
- Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
- Collect physical, psychological, social, and spiritual patient data and incorporate it into plans of care
- Perform initial evaluations and reevaluate patients’ nursing needs during visits
- Conduct follow-up visits according to patient, family, and facility needs
- Communicate significant findings, problems, health-condition changes, environmental issues, and unsafe facility conditions to relevant clinical and care personnel
- Revise plans of care in response to identified patient-care issues and notify relevant stakeholders
- Maintain patient and staff privacy and confidentiality under HIPAA
- Align activities with Elara Caring’s organizational and Home Health leadership vision
Requirements
What you’ll need- Current, unrestricted state RN license
- 1+ year of experience in a clinical care setting as a nurse
- Ability and willingness to travel within the branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Ability and willingness to travel 50%
- Associate’s Degree or Bachelor’s Degree in Nursing preferred
- Previous Home Health or Hospice experience preferred
- Previous experience with HomeCare HomeBase preferred
- Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
- Must be able to provide care in Newaygo and Mecosta Counties
