About the role
Key responsibilities & impact- Assess assigned caseloads of behavioral health care patients and families to identify physical, psychosocial, and environmental needs
- Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating plans of treatment
- Assess patient needs and incorporate physical, psychological, social, and spiritual factors into plans of care
- Perform initial and follow-up evaluations of patients' nursing needs during visits
- Communicate significant findings, problems, and changes in health condition or environment to supervisors, physicians, facilities, and care personnel
- Revise plans of care in response to patient care issues and notify relevant clinical and care team members
- Counsel and instruct patients, facilities, and families regarding the Interdisciplinary Plan of Care
- Teach behavioral health philosophy and pain and symptom management to staff in alternate care settings
- Initiate, supervise, and document behavioral health aide and LPN care delivery according to agency policy
- Report unusual or potentially problematic nursing facility issues
- Provide support at time of death and perform bereavement assessments
Requirements
What you’ll need- High School Diploma or GED
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current, unrestricted state RN license
- 1+ years of experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Ability and willingness to travel within the branch/office coverage area
- Dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Ability and willingness to travel 50%
- Associate's Degree or Bachelor's Degree in Nursing preferred
- Previous Home Health or Hospice experience preferred
- Previous experience with Homecare HomeBase preferred
