About the role

Key responsibilities & impact
  • Provide home health nursing care to patients in their preferred home setting
  • Assess assigned caseloads of home health care patients and families
  • Develop, implement, coordinate, and evaluate patient plans of treatment
  • Assess physical, psychological, social, spiritual, and environmental patient needs
  • Conduct initial evaluations and re-evaluate nursing needs during visits
  • Make follow-up visits according to patient, family, and facility needs
  • Communicate significant findings, problems, health-condition changes, and unsafe conditions to the Clinical Supervisor, physician, facility, and care team
  • Revise plans of care in response to patient-care issues and notify relevant stakeholders
  • Maintain patient and staff privacy and confidentiality under HIPAA
  • Report to the Clinical Manager or Clinical Supervisor

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year of experience in a clinical care setting as a nurse
  • Ability to frequently lift, push, pull, and support up to 50 pounds
  • Ability and willingness to travel within the branch/office coverage area
  • Dependable vehicle
  • Valid driver’s license and current auto insurance in accordance with state laws
  • Ability and willingness to travel 50%
  • Associate’s Degree or Bachelor’s Degree in Nursing preferred
  • Previous Home Health or Hospice experience preferred
  • Previous experience with HomeCare HomeBase preferred
  • Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
