About the role
Key responsibilities & impact- Provide home health nursing care to patients in their preferred home setting
- Assess assigned caseloads of home health care patients and families
- Develop, implement, coordinate, and evaluate patient plans of treatment
- Assess physical, psychological, social, spiritual, and environmental patient needs
- Conduct initial evaluations and re-evaluate nursing needs during visits
- Make follow-up visits according to patient, family, and facility needs
- Communicate significant findings, problems, health-condition changes, and unsafe conditions to the Clinical Supervisor, physician, facility, and care team
- Revise plans of care in response to patient-care issues and notify relevant stakeholders
- Maintain patient and staff privacy and confidentiality under HIPAA
- Report to the Clinical Manager or Clinical Supervisor
Requirements
What you’ll need- Current, unrestricted state RN license
- 1+ year of experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Ability and willingness to travel within the branch/office coverage area
- Dependable vehicle
- Valid driver’s license and current auto insurance in accordance with state laws
- Ability and willingness to travel 50%
- Associate’s Degree or Bachelor’s Degree in Nursing preferred
- Previous Home Health or Hospice experience preferred
- Previous experience with HomeCare HomeBase preferred
- Must maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule
