About the role
Key responsibilities & impact- Ensure activities align with Elara Caring’s vision and Home Health leadership
- Maintain patient and staff privacy and confidentiality under HIPAA
- Assess assigned caseloads of home health patients and families
- Assume primary responsibility for caseloads, including assessing, planning, coordinating, implementing, and evaluating treatment plans
- Assess physical, psychological, social, and spiritual factors affecting patient and caregiver health
- Conduct initial evaluations and re-evaluate nursing needs during visits
- Make follow-up visits according to patient, family, and facility needs
- Communicate significant findings, problems, health changes, environmental concerns, and unsafe facility conditions to relevant personnel
- Revise plans of care in response to patient care issues and notify clinical leadership, physicians, facilities, and team members
- Report to the Clinical Manager or Clinical Supervisor
Requirements
What you’ll need- Current, unrestricted state RN license
- 1+ year of experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Ability and willingness to travel within the branch/office coverage area
- Dependable vehicle
- Valid driver’s license
- Current auto insurance in accordance with state laws
- Ability and willingness to travel 50%
- Associate’s Degree or Bachelor’s Degree in Nursing preferred
- Previous Home Health or Hospice experience preferred
- Previous experience with HomeCare HomeBase preferred
- Ability to maintain HIPAA privacy and confidentiality
