About the role

Key responsibilities & impact
  • Ensure all activities align with Elara Caring’s vision and Hospice leadership
  • Assess hospice patients and families to identify physical, psychosocial, environmental, psychological, social, and spiritual needs
  • Review documentation, clinical records, Interdisciplinary Team reports, after-hours reports, and on-site evaluations
  • Communicate significant findings, problems, health-condition changes, environmental concerns, and unsafe facility conditions to the Clinical Supervisor, physician, facility, and involved care personnel
  • Counsel and instruct patients, facilities, and families on following the Interdisciplinary Plan of Care
  • Teach hospice philosophy and pain and symptom management to staff in alternate care settings
  • Provide appropriate support at the time of death
  • Perform bereavement assessments
  • Report to the Clinical Team Manager

Requirements

What you’ll need
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • Current state license as a Registered Nurse (RN)
  • 1 year of experience in a clinical care setting
  • Experience in a hospice or home health environment preferred
  • 50% travel required
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver’s license and insurance
  • Reliable transportation to perform job tasks
