About the role

Key responsibilities & impact
  • Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment data, and established standards
  • Incorporate patient care goals into nursing visits and document them in skilled nursing notes
  • Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and involved personnel
  • Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
  • Implement patient safety plans using patient, family, and community resources
  • Teach patients appropriate self-care techniques while delivering quality care
  • Suggest revisions to the Plan of Care and notify the RN Case Manager
  • Provide substantial and specialized nursing services under the direction of an RN or physician
  • Collaborate with the RN Case Manager for advice, assistance, and instructions
  • Document medication regimens and update medication profiles
  • Treat patients and caregivers in an effective and compassionate manner
  • Take actions to prevent avoidable hospitalizations and emergency-room visits
  • Provide supportive communication to physicians, patients, families, visitors, and agency personnel
  • Report to the Clinical Team Manager

Requirements

What you’ll need
  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
  • Current state license as a Licensed Vocational / Practical Nurse
  • 1 year of experience in a clinical care setting
  • Experience in a hospice or home health environment is preferred
  • 50% travel required
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver’s license and insurance
  • Reliable transportation to perform job tasks
