About the role
Key responsibilities & impact- Deliver in-home nursing care aligned with patient care plans
- Review and follow the Plan of Care prior to each visit
- Communicate patient updates, concerns, and changes in condition
- Assist with medication management and documentation
- Educate patients and families on self-care techniques
- Identify safety concerns and help prevent hospitalizations
- Collaborate with RN Case Managers, physicians, and the care team
- Report to the Clinical Manager or Branch Director for Personal Care Services
Requirements
What you’ll need- Current LPN/LVN license in the state of Missouri
- Graduate of an accredited nursing program
- At least 1 year of experience in acute care; home care, home health, or hospice preferred
- Strong communication and interpersonal skills
- Ability to manage time effectively and stay organized in the field
- Knowledge of healthcare regulations and patient care standards
- Compassionate, patient-first mindset
- Reliable personal transportation; travel required approximately 50%
- Ability to meet health screening and compliance requirements
