About the role

Key responsibilities & impact
  • Deliver in-home nursing care aligned with patient care plans
  • Review and follow the Plan of Care prior to each visit
  • Communicate patient updates, concerns, and changes in condition
  • Assist with medication management and documentation
  • Educate patients and families on self-care techniques
  • Identify safety concerns and help prevent hospitalizations
  • Collaborate with RN Case Managers, physicians, and the care team
  • Report to the Clinical Manager or Branch Director for Personal Care Services

Requirements

What you’ll need
  • Current LPN/LVN license in the state of Missouri
  • Graduate of an accredited nursing program
  • At least 1 year of experience in acute care; home care, home health, or hospice preferred
  • Strong communication and interpersonal skills
  • Ability to manage time effectively and stay organized in the field
  • Knowledge of healthcare regulations and patient care standards
  • Compassionate, patient-first mindset
  • Reliable personal transportation; travel required approximately 50%
  • Ability to meet health screening and compliance requirements
