About the role

Key responsibilities & impact
  • Ensures that all activities performed align with the vision of Elara Caring
  • Maintains patient and staff privacy and confidentiality
  • Assesses assigned case load of home health care patients and families
  • Assumes primary responsibility for caseload
  • Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit
  • Communicates significant findings, problems, and changes in health condition
  • Revises the plan of care in response to identified patient care issues

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Ability to frequently lift, push, pull, and support up to 50 pounds
  • Must have a dependable vehicle, valid driver’s license, and current auto insurance
  • Associates Degree or Bachelor’s Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
