About the role
Key responsibilities & impact- Managing a caseload of patients and providing comprehensive skilled nursing care in the home environment
- Working closely with physicians and interdisciplinary team members to ensure safe, effective, and individualized care
- Performing comprehensive patient assessments and developing individualized care plans
- Conducting initial and ongoing skilled nursing visits based on patient needs and physician orders
- Monitoring and evaluating patient status, including physical, psychosocial, environmental, and caregiver factors
- Coordinating care with physicians, therapists, social workers, and other care team members
- Communicating changes in patient condition and recommending appropriate interventions
- Updating and revising care plans as patient needs evolve
- Providing patient and caregiver education to support positive outcomes
- Maintaining timely, accurate clinical documentation in compliance with regulations and company standards
- Ensuring adherence to HIPAA and patient confidentiality requirements
Requirements
What you’ll need- Current, unrestricted RN license in the applicable state
- Minimum of 1 year of nursing experience in a clinical setting
- Valid driver’s license, auto insurance, and reliable transportation
- Willingness to travel within the assigned branch territory (approximately 50%)
- Ability to sit, stand, bend, lift, and move intermittently
- Ability to lift 50–100 lbs as needed
