About the role

Key responsibilities & impact
  • Managing a caseload of patients and providing comprehensive skilled nursing care in the home environment
  • Working closely with physicians and interdisciplinary team members to ensure safe, effective, and individualized care
  • Performing comprehensive patient assessments and developing individualized care plans
  • Conducting initial and ongoing skilled nursing visits based on patient needs and physician orders
  • Monitoring and evaluating patient status, including physical, psychosocial, environmental, and caregiver factors
  • Coordinating care with physicians, therapists, social workers, and other care team members
  • Communicating changes in patient condition and recommending appropriate interventions

  • Ensuring adherence to HIPAA and patient confidentiality requirements

Requirements

What you’ll need
  • Current, unrestricted RN license in the applicable state
  • Minimum of 1 year of nursing experience in a clinical setting
  • Valid driver’s license, auto insurance, and reliable transportation
  • Willingness to travel within the assigned branch territory (approximately 50%)
  • Ability to sit, stand, bend, lift, and move intermittently
  • Ability to lift 50–100 lbs as needed
