About the role

Key responsibilities & impact
  • Provide compassionate, one-on-one care in patients’ homes
  • Manage a caseload of patients in their homes, providing skilled nursing care
  • Care coordination, patient education, and ongoing assessments
  • Collaborate closely with physicians, caregivers, and interdisciplinary team members
  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Monitor patient conditions and identify impacting health factors
  • Coordinate care with physicians, therapists, social workers, and other healthcare team members
  • Communicate significant changes in patient condition and recommend appropriate interventions
  • Update and revise care plans as patient needs evolve
  • Provide education and support to patients, families, and caregivers
  • Maintain accurate, timely clinical documentation and ensure compliance with regulatory and company standards

Requirements

What you’ll need
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Valid driver’s license and insurance and reliable transportation to perform job tasks
  • Able and willing to travel within the branch/office coverage area
  • Must be able and willing to travel 50%
  • Ability to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs
