About the role

Key responsibilities & impact

Provide compassionate, one-on-one care in patients’ homes
Manage a caseload of patients in their homes, providing skilled nursing care
Care coordination, patient education, and ongoing assessments
Collaborate closely with physicians, caregivers, and interdisciplinary team members
Conduct comprehensive patient assessments and develop individualized plans of care
Monitor patient conditions and identify impacting health factors
Coordinate care with physicians, therapists, social workers, and other healthcare team members
Communicate significant changes in patient condition and recommend appropriate interventions
Update and revise care plans as patient needs evolve
Provide education and support to patients, families, and caregivers
Maintain accurate, timely clinical documentation and ensure compliance with regulatory and company standards

Requirements

What you’ll need

Current, unrestricted state RN license
1+ year experience in a clinical care setting as a nurse
Valid driver’s license and insurance and reliable transportation to perform job tasks
Able and willing to travel within the branch/office coverage area
Must be able and willing to travel 50%
Ability to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs
