About the role

Key responsibilities & impact

Obtains a medical history, particularly as it relates to the present condition, from the member and/or family member(s).
Evaluates the client’s condition and home situation to determine if other services will be required and the frequency of this service.
Surveys the client’s environment to determine what assistance will be available from family members and other informal supports in caring for the client.
Assesses the client’s family member(s), home situation, to determine if other services or health teaching will be required and the frequency of this services.
Interprets nursing and other services to clients and their families as a part of planning for care.
Develops the client’s service plan and coordinates the authorized services.
Monitors the effectiveness of the service plan per Company policy and State and Federal Program regulation.
Submits case management progress notes and other documents outlining the services rendered as indicated.
Submits a tally of visits made each day.
Participates in case conferences and staff development meetings.
Discusses with the supervisor problems concerning the clients and how they may best be handled.
Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement the client’s service plan.

Requirements

What you’ll need

High School Diploma/GED
1+ year RN or LPN experience
Must have a dependable vehicle, valid driver’s license and current auto insurance in accordance with the state.
Proficiency with MS Office Suite
Previous experience with Workday is preferred
