About the role
Key responsibilities & impact- Manage a consistent caseload from admission through discharge
- Complete Start of Care, OASIS, recertifications, and routine visits
- Develop, implement, and adjust patient-centered care plans
- Identify clinical changes early and intervene appropriately
- Educate patients and caregivers to improve independence
- Collaborate with physicians and interdisciplinary teams
- Document efficiently using HomeCare HomeBase
Requirements
What you’ll need- Active RN license
- 1+ year clinical nursing experience
- Home Health or Hospice experience preferred
- Strong assessment, organization, and time management skills
- Ability to work independently and make clinical decisions
- Reliable transportation and willingness to travel within territory
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- HomeCare HomeBase experience is a plus
