About the role

  • Promote Elara Caring’s mission, philosophy, and standards of excellence
  • Assist in implementing individualized rehabilitation plans under the direction of an Occupational Therapist
  • Perform routine patient visits and reassess therapy needs at each visit
  • Communicate significant findings or changes in patient condition to the OT, physician, and care team
  • Verify and follow the established Plan of Care and physician orders
  • Educate patients and caregivers on therapy goals, home exercise programs, and safety
  • Ensure compliance with Medicare, Medicaid, and third-party payer requirements
  • Identify and report potential coverage or payment concerns
  • Maintain timely, accurate clinical documentation
  • Collaborate with clinical staff on schedules, patient needs, and care coordination
  • Support initiatives to prevent avoidable hospitalizations and ER visits

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program
  • Current, unrestricted COTA license in the state of practice
  • Minimum 1 year of clinical experience as a COTA
  • Ability and willingness to travel within the branch coverage area (up to 50%)
  • Valid driver’s license, reliable vehicle, and current auto insurance
  • Ability to sit, stand, bend, lift, and move throughout the workday
