About the role
- Promote Elara Caring’s mission, philosophy, and standards of excellence
- Assist in implementing individualized rehabilitation plans under the direction of an Occupational Therapist
- Perform routine patient visits and reassess therapy needs at each visit
- Communicate significant findings or changes in patient condition to the OT, physician, and care team
- Verify and follow the established Plan of Care and physician orders
- Educate patients and caregivers on therapy goals, home exercise programs, and safety
- Ensure compliance with Medicare, Medicaid, and third-party payer requirements
- Identify and report potential coverage or payment concerns
- Maintain timely, accurate clinical documentation
- Collaborate with clinical staff on schedules, patient needs, and care coordination
- Support initiatives to prevent avoidable hospitalizations and ER visits
Requirements
- Graduate of an accredited Occupational Therapy Assistant program
- Current, unrestricted COTA license in the state of practice
- Minimum 1 year of clinical experience as a COTA
- Ability and willingness to travel within the branch coverage area (up to 50%)
- Valid driver’s license, reliable vehicle, and current auto insurance
- Ability to sit, stand, bend, lift, and move throughout the workday
