  • Conducting SLP evaluations to determine patient needs
  • Completing OASIS-driven assessments and follow-up visits
  • Developing personalized Plans of Care
  • Ensuring patient safety and engaging family/caregiver resources

  • Providing direct therapy using accepted, evidence-based practices

Requirements

  • Master’s degree in Speech-Language Pathology
  • Current, unrestricted Indiana SLP license
  • Minimum 1 year of clinical experience
  • Ability to travel within the Terre Haute service area
  • Reliable vehicle, valid driver’s license & auto insurance
  • Ability to lift 50–100 lbs and perform physical tasks as needed
  • Willingness to travel up to 50% depending on caseload
