About the role
- Promoting Elara Caring’s mission & high‑quality standards
- Conducting SLP evaluations to determine patient needs
- Completing OASIS-driven assessments and follow-up visits
- Developing personalized Plans of Care
- Ensuring patient safety and engaging family/caregiver resources
- Communicating care updates with physicians and interdisciplinary teams
- Documenting accurately & timely
- Providing direct therapy using accepted, evidence-based practices
- Teaching patients/caregivers home exercise programs
- Participating in consistent care coordination
Requirements
- Master’s degree in Speech-Language Pathology
- Current, unrestricted Indiana SLP license
- Minimum 1 year of clinical experience
- Ability to travel within the Terre Haute service area
- Reliable vehicle, valid driver’s license & auto insurance
- Ability to lift 50–100 lbs and perform physical tasks as needed
- Willingness to travel up to 50% depending on caseload
