About the role
- Deliver high-quality, patient-centered physical therapy in the home setting
- Evaluate patient needs, develop individualized plans of care, and adjust treatment to support optimal outcomes
- Collaborate with interdisciplinary team members to ensure coordinated care
- Educate patients and caregivers to support continued progress at home
- Provide leadership and guidance to support staff when needed
- Maintain timely, accurate documentation in accordance with standards of care
- Serve as a positive ambassador for Elara Caring within the community
Requirements
- Graduate of an APTA-accredited Physical Therapy program
- Active, unrestricted PT license in the state of practice
- At least 1 year of experience in a healthcare setting
- Comfortable working independently in a field-based role
- Willing and able to travel up to 50% within the coverage area
- Reliable vehicle, valid driver’s license, and current auto insurance
- Ability to perform physical demands of the role, including lifting up to 50–100 lbs
